Who may apply
Anyone 21 or older may apply, however all information must be completed by the applicant. No other person can complete the information on behalf of the applicant.

Information you will need prior to beginning this online application, are as follows:

1. Background and Job information. If you have any derogatory remarks on your background, you must provide copies of documents and written explanation for such remarks.

2. Your Social Security Number. By Federal Law, MFS must maintain social security numbers for persons to whom MFS makes disbursement. The number you provide must correspond to your legal last name . Company name is not allowed.

3. Three Character references with name and phone numbers who can verify that you are of good moral character

4. Credit Card Information. You will be asked to pay a non-refundable $150 administrative services fee as part of the application process. Please have an AMEX, Discover, Visa or Master Card available. The name and address information on the credit card must match your name and address.

Please Note: If someone else authorizes you to use their credit card for this transaction, you MUST immediately contact MFS membership Department at 404-286-6908, so that we can get written authorization from them, allowing you to use their credit card. Please be informed that unauthorized use of a credit card may be considered fraud. We have zero tolerance for fraud.
